Specialized Family Care Program

SFC staff will be working remotely and available to SFC providers through their work cell phones. If you have an emergency or specific needs please contact your Family Based Care Specialist. The public may continue to make referrals or apply to be a home provider by utilizing this site. All staff contact information can be accessed by Contact Us.

  2. Specialized Family Care Provider knows about the historical court decision in WV which improved the lives of persons with disabilities
  3. Specialized Family Care Provider demonstrates sensitivity to struggles of persons with disabilities

  • Test completed by Provider (Take the Test Below)
  • Review of test responses by Family Based Care Specialist and Provider
