Specialized Family Care Program

SFC staff will be working remotely and available to SFC providers through their work cell phones. If you have an emergency or specific needs please contact your Family Based Care Specialist. The public may continue to make referrals or apply to be a home provider by utilizing this site. All staff contact information can be accessed by Contact Us.

Training Objectives:

  2. Provider knows the signs of CHF
  3. Provider knows about common medications used to treat CHF
  4. Provider knows about diet and other wellness issues related to CHF

Training Procedures:

  • *Provider initiated self-study. Alternate formats of the study materials are available upon request. (Course Materials: Cover Page and Part I Part II)
  • Test completed by Provider (Take the Test Below)
  • Review of test responses by Family Based Care Specialist and Provider
