Steps to Become a Provider

1. Submit inquiry form online at sfcp.cedwvu.org
2. Introductory phone interview or home visit with SFC staff to learn more
3. Complete application and provide references
4. Home visit assessment
5. Background checks and medical clearance
6. Complete training
7. Final walk through
8. Paperwork processing

This program is an interagency collaboration between the West Virginia Department of Health & Human Resources and the Center for Excellence in Disabilities. The SFC Program is funded under an agreement with the West Virginia Department of Health & Human Resources, Bureau for Children & Families.
